Our Survey will give us a better idea of who you are. Yes it's a simple application form, but we only are looking for people who are ready for change and this helps us gauge your readiness. If you can't be bothered to spend the time to fill it out it's an early indicator to us that maybe now is not an ideal time to move forward with our program
